Dear Mr. Wagner:
On the recommendation of the Help Desk Manager of useIT, we are pleased to offer you an appointment as Administrative Intern for User Services and Instructional Technology for the academic year 2003-2004.
In return for your services as Administrative Intern, you would receive compensation as follows:
1. A salary of $9,840.00 for the period of service May 14, 2003, through May 13, 2004. Your salary will be paid in twelve monthly installments on the 25th of each month (or the last working day before, if the 25th falls on a weekend or holiday) beginning May 23, 2003.
2. Residence in College housing. By signing this contract, you agree to live in housing available for the period beginning May 13, 2003, through May 20, 2004, and to vacate the housing by the latter date. Please contact Jacqueline Middleton () to discuss your housing arrangement.
3. Interns may eat in College dining facilities. Each intern is permitted 21 meals per week (3 meals per day) in Lowry Center or Kittredge dining halls or breakfast at the Java Hut in the Lowry Center lower promenade. When Lowry and Kittredge dining halls are closed, interns are responsible for their own meals. Department per diems are not permitted. You are required to present your College I.D. to eat meals in all of the locations. Meals are available to you from May 13, 2003, through May 20, 2004. However, because you will be required to work occasionally outside the campus dining hours, the department will provide two payments ($150.00 in December and $150.00 in March) as a taxable addition to your salary.
4. Interns have the right to enroll in one course of their choice per semester, without charge. Private instruction courses are not included in this offering.
5. Related medical benefits: the services at the Student Wellness Center will be extended to you on the same basis as they are to students, but the regular fees will be waived. The College will cover the cost for Part I and Part II (the extended or hospital coverage) of the Student Health Insurance plan. Please inform Kathy Breitenbucher if you need the Student Health Insurance, and then make the necessary arrangements with Joyce Hansen in the Business Office. If you do not need health insurance, please complete a waiver form available at the Student Wellness Center and inform the Office of the Vice President for Academic Affairs.
This offer is contingent upon the satisfaction of employment authorization and verification of identity as required by the Immigration Reform and Control Act of 1986, as amended. Within three days from the effective date of hire, you will need to provide the College with documentation establishing both identity and employment eligibility. Acceptable documents are listed on the attached page; by law, originals or certified copies are required. These need to be presented at The Human Resources Office, located on Wayne Avenue next to the Longbrake Wellness Center. Please call the Office directly at 330-263-2526 with any questions about this requirement.

The general terms of the Internship Program for 2003-2004 are described on the enclosed sheet.
Your duties will be determined and supervised by the Help Desk Manager. You will be required to participate fully in the orientation and educational programs for interns arranged by the Office of the Dean of Students. These meetings with all interns will include discussions of job experiences and of the practical and theoretical aspects of college administration.
Although your contract is anticipated to extend for the full twelve-month term, your employment will be subject to termination for failure to perform your duties in a manner deemed satisfactory by the Help Desk Manager. Experience has made it necessary for us to add to the contract the agreement that as a resident in College property you recognize the College's policy against pets in College residences.

Date: 2004-05-24 10:56 am (UTC)Get the following together.
1. A copy of the contract signed by you
2. Pay stubs - especially that last one
3. Some sort of paperwork noting what you final day of work was.
4. A letter explaining the problem in detail and asking for the rest of your pay.
Take or mail all of that to the payroll people somehow (Make sure you give them copies and keep the originals for yourself) and make your demand for payment again. See what they do. Come back and report in.
Hope this helps.
